External Activities Forms for Homeschoolers

There are certain basic forms that are used for recording outside activities like field trips or homeschool meetings. These types of homeschool activities forms include the Field Trip Log that can be used for keeping a record of the field trips. The form provides space at the top for writing the name, grade and academic year of the student. The form is divided into two columns.


The first column is for mentioning the educational purpose, contact person, location and cost and the other column is for mentioning the date or any other notes. The form is divided into three spaces and provides a space at the bottom for the purpose of writing notes.

The Extracurricular Activities Form is used for recording any kind of extracurricular activity and the time spent on those activities. The form consists of four columns for mentioning the name of the activity, time spent on each activity, the total amount of time spent and the date. At the top, there is space for mentioning the name, grade and academic year of the student.

The Important Websites Form can be used for recording the user names, passwords and website URL’s. Since this form is supposed to be at your home only, it should be kept in a secure place. It provides space for storing information about 16 websites. Each space is divided into two columns. While the first column is meant for writing the name of the website, the second column is further divided into two rows for writing the user name and the password.

The Meeting Notes Form is useful for taking notes of the homeschool meetings. This form is basically a ruled page, in which the space for writing the name of the person with whom you would meet, place, time of meeting and date of the meeting is situated at the top.
The Contact Information Form is meant for keeping the phone numbers, URL’s, email addresses and names of the children of the homeschoolers. The page is divided into four spaces. Each space is further divided into two columns. The left column is for writing the name, address, website, email and children’s names and the right-hand side is for writing the contact numbers, including the home, office and mobile numbers.
