Various Kinds of Book List Forms
Here are a few Homeschool book List Forms, which you can design:
The Book Reading List comprises three columns. The first column provides space for writing the name of the book and the author. The second column provides space for checkmark, which indicates whether that book has been read or not and the third column is for mentioning the date. The Reading Form also provides space at the top, for writing the name of the student.
The Book Rating List consists of five columns for mentioning the title of the book, author of the book, the initials of the Reader, the Rating and the date. Here the reader can rate the book, within a mark of 1 to 10, with 5 being the average and 10 being best.
The Book Reading Log is capable of recording the details of as many as ten books. The two sheets provide space for recording the details of ten Magazines, Books or Articles. There are columns for mentioning the date, name of book and opinions. The space for writing the name, year and grade of the student is situated at the top of the page. At the bottom, there is a space for writing notes.
The Library Books List consists of five columns. The first column is for the title of the book, the second column is for author of the book, the third column mentions the phone number, the fourth column mentions the due date and the fifth column is for the checkmark to indicate whether the book has been returned or not.
The Chapter Book List is basically meant for one book. The readers can use this form to comment on each chapter of the book. It is very useful in preparing book reports and analysis. Its design enables a student to comment on a maximum of 35 chapters. At the top, there are provisions for writing the title and author of the book. The page is divided into three columns. While the first column provides space for writing the Chapter No., the second column is used to write the title while the third column provides space for writing comments. At the bottom, there is space for writing the date of having completed the book, notes and signature.
For the high school goers, two types of forms are available. The first form is a “The Reading List” of the students. In this form the student can write his/her name, academic year and grade. This list has four columns representing date, title, author and Fiction/Non-Fiction.
The second form with a heading, “Reading List for Journals and Articles” consists of three columns. The first column is for mentioning the date, the second column is for mentioning name of the periodical or journal or issue and the third column is for mentioning the article. It also provides space at the top for the name, grade and academic year of the student.
